MAGCOR Reentry Division looking to hire Facilities-Based Case Manager
The Facilities-Based Case Manager is responsible for providing pre-release case management services to incarcerated individuals inside Yazoo Federal Prison. This role focuses on preparing adults in custody (AICs) for reentry by helping them develop personalized transition plans, connecting them with resources, and coordinating with external partners to ensure support is in place upon release. The ideal candidate is someone who can work collaboratively in a secure environment, navigate institutional processes, and build trusting relationships with clients. This position requires clearance through an NCIC background check and authorized access to correctional facilities.

Key Responsibilities:
Client Support and Pre-Release Planning:
• Meet with clients inside correctional facilities to assess reentry needs, develop individualized transition plans, and identify goals related to employment, housing, education, and health.
• Provide guidance on obtaining identification, social security cards, and necessary documentation prior to release.
• Connect clients with training opportunities, certification programs, and job readiness resources available during incarceration.
• Educate clients about available community-based services and coordinate warm handoffs to community case managers and support partners. Collaboration and Coordination:
• Serve as the key liaison between facility staff, MAGCOR reentry programs, and external service providers.
• Work closely with prison counselors, reentry coordinators, and parole staff to ensure a smooth transition process.
• Support family reunification efforts and help clients begin preparing for reintegration into their home communities.
Documentation and Data Tracking:
• Maintain accurate and timely records of client progress, assessments, service plans, and referrals in a case management system.
• Track outcomes related to readiness, participation, and post-release referrals.
• Attend team meetings and contribute to continuous improvement efforts for facility-based reentry services.
Qualifications:
• Bachelor’s degree in Social Work, Criminal Justice, Psychology, Human Services, or a related field preferred, OR equivalent combination of education and 2–5 years of relevant experience in reentry services or corrections.
• Strong knowledge of reentry barriers, institutional systems, and support services for justice-impacted individuals.
• Must pass an NCIC background check and be eligible for facility access under Mississippi Department of Corrections and/or Federal Bureau of Prisons guidelines.
• Excellent interpersonal skills and the ability to build rapport in a structured, secure setting.
• Experience working in or with correctional institutions is highly valued.
• Ability to work independently, follow facility protocols, and manage a caseload across multiple institutions if needed.
• Strong written documentation, organizational, and time-management skills.
